Case Manager Full time – responsible for coordinating case management services for the Certified Apartment Program clients, assist clients in applying for, coordinate and pursue benefit entitlements, providing supportive, goal-oriented counseling with a focus on decision-making and problem solving skills; and developing and implementing service plans.  Advocate with or on behalf of client’s to other agencies to ensure access to needed service. In addition, complete fee agreements, fee schedules, and financial status and client ledger sheets.  Record necessary financial information and file all benefit correspondence.  Communicate regularly with Finance Dept. to review and reconcile clients’ accounts. Qualifications: Associate’s Degree in Human Services, 1 year experience in case management, ability to clearly communication both orally and in writing, ability to be flexible, have excellent organizational skills and the ability to follow through with multiple tasks, and computer experience.  Must have a valid and insurable driver’s license.  Schedule: Monday-Friday, daytime.  $16.62/hour; $34,569.60/annual equivalent.

 

Case Manager- Housing Coordinator Full Time: – Provide case management services; securing safe and affordable housing; teaching the necessary skills to maintain stable housing; developing linkages to community services; and develop support plans for adults who have a diagnosis of HIV/AIDs, mental illness, and mentally ill adults who need intensive support and engagement in services, (MRT).  College degree in human services and/or equivalent experience in the human service field especially in the populations diagnosed with HIV/AIDs, mentally illness, substance abuse, and/or chronic homelessness.  In addition, case management experience and a valid and insurable driver’s license are required. 40 hours per week.  $15.68/hour.  $32,641/ annual equivalent.  Schedule: Monday-Friday, 8:30-4:30.  No after hours on-call.

 

Service Coordinator Full-Time: – Providing supportive counseling to adults who have a psychiatric diagnosis and who live in independent apartments in the Schenectady community.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $15.51/hour; $32,260.80/annual equivalent. Schedule-Monday-Friday, day time hours.  On-Call for Client Emergencies on a rotating bases for 1 week; $150 bonus for on-call.

 

Dominion House Residence Counselor – Full Time– Providing supportive counseling to adults who have a psychiatric diagnosis and who live in a short-term stabilization program (group home).  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $14.90/hour; $30,992.00/annual equivalent.

Residence Counselor–Full Time Emmanuel House– Provide supportive counseling to adults with a psychiatric diagnosis living in a group home setting.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; demonstrated ability in helping others, valid and insurable driver’s license. $14.28/hour; $29,720/annual equivalent.

Relief Counselor/Respite – Per Diem

We are looking for everyday heroes who want to make a difference in the lives of people in need!

Mohawk Opportunities, a small private, nonprofit located in Schenectady is seeking per diem (substitute) Relief Counselors to work in our 4 group homes. We serve adults who have a mental illness and are working on goals to transition to an independent setting. The clients we work with are very high functioning and are in the community independently. We DO NOT provide personal care and we DO NOT do physical restraints. All the services to our clients are verbal supportive counseling and teaching of daily living skills. We provide personal protective equipment and follow all CDC guidelines. You must be available for all shifts. Driver’s license is not required but preferred. Counselors must be willing to work in the group home if there is a COVID exposure. Regular rate $14.28/hour and the rate will increase to time and a half, $19.62/hour if there is a COVID exposure. We provide training and a supportive work environment.
