Social Worker-We are seeking a Social Worker to be a clinical staff member of the ACT community based multidisciplinary treatment team. Services provision is designed to promote recovery by improving psychiatric symptoms, preventing relapse, teaching skills, providing direct assistance and securing necessary community resources. Duties include: function as the lead for vocational and educational services; offer assistance in all phases of educational and vocational development programming including vocational assessment, job exploration, and skills development as they seek to enroll in an educational/vocational program and/or obtain and maintain employment. Use strength based methodologies, continually instill hope, develop and maintain trusting and supportive relationships with clients, families, other providers and the community. This is a community based positon which includes meeting with clients in their homes as well as provide on-call crisis intervention services on a rotation with other team members. Minimum qualifications include: Master’s Degree in Social Work with licensure, hold a license as a Mental Health Counselor in New York State, Master’s degree in Rehabilitation Counseling, Certification from The Commission on Rehabilitation Counseling. Three years of experience working in an inpatient or outpatient psychiatric setting preferred. A valid and insurable NYS driver’s license is also required. This position is a Monday through Friday day time position with rotating on-call when the office is closed. There is a $150 weekly stipend for the rotating on-call. Care Manager (Case Manager) for Care Coordination – We are seeking a candidate who demonstrates initiative, confidence, and a focus on making positive connections to coordinate care management services by integrating, overseeing and monitoring all aspects of the physical and behavioral health needs of Health Home Clients. Engage clients and assess eligibility for care management. Complete comprehensive assessments; develop and monitor interdisciplinary plan of care aimed at achieving stability within the community; provide referrals and linkages to community services; regularly communicate with other care providers to exchange information and identify needs; provide outreach to those that are in need but not currently receiving service and coordinating with Managed Care Companies. Min qual.: BA/BS in Human Services and 2 years human services exp. OR BA/BS in another field with 5 years human service exp; 1 year in case mgt (defined as experience in assessment of client’s need, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services); a demonstrated ability to function as a member of an interdisciplinary team, knowledge of mental illness/behavioral health, substance abuse, physical/medical health, and homelessness is required, and a valid and insurable NYS driver’s license. 1yr’s exp. working in a health care setting providing direct care to clients, preferred. If no applicants meet the required minimum qualifications, a waiver of these qualifications might be available from the regulatory agency; therefore we encourage those close to the minimum qualifications to apply. $17.60/hour. $36,608/ annual equivalent. 40 hrs/week. Schedule: Monday-Friday, daytime hours.
Residence Counselor–Full Time and Part Time – Provide supportive counseling to adults with a psychiatric diagnosis living in a group home setting. Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent. Qualifications include college classes in human services, BA preferred; demonstrated ability in helping others, valid and insurable driver’s license. $14.00/hour; $29,120/annual equivalent.
