Assistant Director of Administrative and Program Support Duties – We are seeking a detailed, organized professional person to provide administrative support to all levels of the agency. The position is an executive salaried position. Duties include working directly with the Board of Directors; assisting the Executive Director with coordination of tasks, new initiatives, and projects; fundraising and public relations, program office support, administrative office support, facilities oversight and coordination.  Qualifications include: 5 years’ experience supporting executive level administration, preferably in a non-profit organization supporting people with disabilities; office management/administration experience, event planning exp, working with various types of vendors, supply procurement, and supervising reception. Experience working with boards of directors preferred, but not required. Must be experienced with the various Microsoft Windows programs. High School Diploma required; college degree preferred. Care Manager for the Health Home Plus Program– We are seeking a candidate who demonstrates initiative, confidence, and a focus on making positive connections to coordinate care management services by integrating, overseeing and monitoring all aspects of the physical and behavioral health needs of Health Home Clients.  Engage clients and assess eligibility for care mgt and Home and Community Based Services. Complete comprehensive assessments; develop and monitor interdisciplinary plan of care aimed at achieving stability within the community; provide referrals and linkages to community services; regularly communicate with other care providers to exchange information and identify needs; provide outreach to those that are in need but not currently receiving service and coordinating with Managed Care Companies.  Min qual.: BA/BS in Human Services and 2 years human services exp. OR BA/BS in another field with 5 years human service exp; 1 year in case mgt (defined as experience in assessment of client’s need, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services); a demonstrated ability to function as a member of an interdisciplinary team, knowledge of mental illness/behavioral health, substance abuse, physical/medical health, and homelessness is required, and a valid and insurable NYS driver’s license. 1 year’s exp. working in a health care setting providing direct care to clients is preferred.  $19.31/hour.  $40,164.80/annual equivalent.  40 hours/week. Dominion House Case Manager – Full Time

Provide service plan development and case management services to clients at Dominion House.  Coordinate all collaborative meetings with counselors and clients to facilitate residential treatment plan meetings then write individual service plans using the a client centered approach.  In addition, provide case management services to clients including benefit entitlements.  Qualifications include BA in human services or combination of degree and experience.  Excellent written skills, communication skills, and computer skills required along with mental health exp.  A valid and insurable driver’s license also required.  40 hours per week.  $18.12/hour.  $37,689.60/annual equivalent.  Overtime is available; $27.18/overtime pay rate.

Residence Counselor- Full Time –

Provide supportive counseling to adults with a psychiatric diagnosis living in a group home setting.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; demonstrated ability in helping others, valid and insurable driver’s license. $15.78/hour; $32,822.40/annual equivalent. Overtime is available in our group home setting, $23.67/overtime pay rate.

Dominion House Residence Counselor – Full Time– Providing supportive counseling to adults who have a psychiatric diagnosis and who live in a short-term stabilization program (group home).  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $16.40/hour; $34,112.00/annual equivalent.  Overtime is available; $24.60/overtime pay rate.

Relief Counselor/Respite – Per Diem

We are looking for everyday heroes who want to make a difference in the lives of people in need!

Mohawk Opportunities, a small private, nonprofit located in Schenectady is seeking per diem (substitute) Relief Counselors to work in our 4 group homes. We serve adults who have a mental illness and are working on goals to transition to an independent setting. The clients we work with are very high functioning and are in the community independently. We DO NOT provide personal care and we DO NOT do physical restraints. All the services to our clients are verbal supportive counseling and teaching of daily living skills. We provide personal protective equipment and follow all CDC guidelines. You must be available for all shifts. Driver’s license is not required but preferred. Counselors must be willing to work in the group home if there is a COVID exposure. Regular rate $15.78/hour and the rate will increase to time and a half, $23.67/hour if there is a COVID exposure. We provide training and a supportive work environment.
