Mohawk Opportunities is seeking caring and dedicated professionals who enjoy working for a small nonprofit.

Registered Nurse ( RN ) – Residential Services is to serves as a resource for the clients in Mohawk Opportunities’ four Community Residences, the Certified Apartment Program and the Supported Housing Program ensuring that significant medical needs are appropriately addressed.  The RN will follow-up and interface with medical providers as needed to monitor that appropriate follow-up care is received and will consult with program staff so as to ensure they are aware of and prepared to support follow-up care as needed.  The RN is expected to meet with clients in their homes/community and transport clients as necessary to medical appointments in the community.  Minimum qualifications include: NYS Licensed Registered Professional Nurse; Bachelor’s degree in nursing, preferred; three years’ experience in a psychiatric setting with at least one year’s experience in a residential setting, preferred; computer experience preferred; valid and insurable driver’s license.  40 hours per week. Monday-Friday. DAY TIME HOURS AND NO ON-CALL. Salary commensurate with experience.  Please state salary requirement in cover letter.

Health Home Plus Care Manager (Case Manager) – Responsible for coordinating, integrating, overseeing and monitoring all aspects of the physical, behavioral and social health needs of the High-Need Seriously Mentally Ill (SMI) population.   Complete enrollment and ongoing assessments;  provide weekly intensive case management service to high-needs clients, develop and monitor interdisciplinary plan of care reflecting an increased intensity of needs aimed at achieving stability within the community; provide referrals and linkages to community services; regularly communicate with other care providers to exchange information and identify needs; provide outreach to those that are in need but not currently receiving service and coordinating with Managed Care Companies.  Min. qualification include: BA/BS in Human Services, minimum of 2 years’ human services or a Master’s Degree with 1 year’s exp; 1 year in case management (defined as experience in assessment of client’s need, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services); knowledge of mental illness, substance abuse, physical/medical health, and homelessness is required, and a valid and insurable NYS driver’s license.  1year’s experience working in a health care setting providing direct care to clients is preferred.  $17.79/hour.  $37,003/ annual equivalent.  40 hours/week. Schedule: Monday-Friday daytime hours.  Some flexibility with early morning hours as clients might have early morning medical appointments.

Service Coordinator Full-Time – Providing supportive counseling to adults who have a psychiatric diagnosis and who live in independent apartments in the Schenectady community.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $14.87/hour; $30,929/annual equivalent. Schedule-Monday-Friday, day time hours.  On-Call for Client Emergencies on a rotating 1 week bases (once every 3 months).

Case Manager – Housing Coordinator – Provide case management services; securing safe and affordable housing; teaching the necessary skills to maintain stable housing; developing linkages to community services; and develop support plans for adults who have a diagnosis of HIV/AIDs, mental illness, and mentally ill adults who need intensive support and engagement in services, (MRT).  College degree in human services and/or equivalent experience in the human service field especially in the populations diagnosed with HIV/AIDs, mentally illness, substance abuse, and chronic homelessness.  In addition, case management experience and a valid and insurable driver’s license are required. 40 hours per week.  Monday-Friday.  Daytime hours.  No after hours on-call.  $15.02/hour.  $31,241/ annual equivalent plus benefits, staff training, and paid time off.

Dominion House Residence Counselor Full Time- Providing supportive counseling to adults who have a psychiatric diagnosis and who live in a short-term stabilization program (group home).  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $14.28/hour; $29,702/annual equivalent.

Schedule 1:  Must have availability for all shifts.  Schedule will change depending on the program needs and with 2 weeks’ notice.

 Residence Counselor–Full Time and Part Time – Provide supportive counseling to adults with a psychiatric diagnosis living in a group home setting.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; demonstrated ability in helping others, valid and insurable driver’s license.  $13.69/hour; $28,475.20/annual equivalent.
