Care Manager for the Health Home Plus Program– We are seeking a candidate who demonstrates initiative, confidence, and a focus on making positive connections to coordinate care management services by integrating, overseeing and monitoring all aspects of the physical and behavioral health needs of Health Home Clients.  Engage clients and assess eligibility for care mgt and Home and Community Based Services. Complete comprehensive assessments: develop and monitor interdisciplinary plan of care aimed at achieving stability within the community; provide referrals and linkages to community services; regularly communicate with other care providers to exchange information and identify needs; provide outreach to those that are in need but not currently receiving service and coordinating with Managed Care Companies.  Min qua.: BA/BS in Human Services and 2 years human services exp. OR BA/BS in another field with 5 years human service exp; 1 year in case mgt (defined as experience in assessment of client’s need, care planning, implementation of care plan and regular review along with benefit entitlements, linkage to community providers and services); a demonstrated ability to function as a member of an interdisciplinary team, knowledge of mental illness/behavioral health, substance abuse, physical/medical health, and homelessness is required, and a valid and insurable NYS driver’s license.  1year’s exp. working in a health care setting providing direct care to clients is preferred.  $23.50/hour. $48,880.00/year. 40 hours/week. Schedule: Monday-Friday daytime hours.  Some flexibility with early morning hours as clients might have early morning medical appointments.

Dominion House Overnight Counselor, Part Time- Provide a safe and therapeutic environment for adults who have a psychiatric diagnosis and who are recovering in a short-term stabilization program (group home). Duties include supportive counseling, teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent. Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license preferred but not required. 21 hours per week. $21.12/hour; $23,063.04/annual equivalent.

ACT Team Counselor – Part Time: A member of the ACT multidisciplinary team, and has the experience to assess, plan, develop, coordinate, and provide treatment and support services to program clients with a diagnosis severe mental illness.  These services will be provided in the client’s home, community, or office.  Duties include comprehensive assessment with supervisor guidance; collaboration with ACT Team staff members, develop and implement person-centered individual treatment plans; case management services; supportive counseling; and cofacilitate therapeutic groups. Min qual:  BA/BS in human services OR an associate degree in human services with a CASAC or other relevant credentials.  Three years’ experience in human service. Specific experience in mental health or substance use disorder preferred. A valid and insurable driver’s license.  $30.00/hour; 20 hours per week; $31,200/annual equivalent; prorated fringe benefits.  Work hours are within the client service hours of Monday-Thursday, daytime hours. Must attend morning team meeting (9:00am-10:30am) twice per week at a minimum.

ACT Team Psychiatric Nurse – Part Time–The Assertive Community Team (ACT) Psychiatric Nurse is a clinical staff member of the ACT community based multidisciplinary treatment team.  Provide assistance to the full time Nurse in the following areas: client’s medication management and administering Long-Acting Injectable medications. Conduct health assessment and collect medical history.  Assist with linkage to medical providers.  In collaboration with ACT Team clinicians, respond to crisis situations, as needed. Facilitate client recovery, personal growth; manage symptoms, gain independence, and master use of community resources to the best of each client’s ability. This is a community-based position which includes meeting with clients in their homes, community or in the office in addition to providing transportation to appointments using agency provided vehicles.  Min qual:  Licensed Registered Professional Nurse, and A valid and insurable driver’s license.  Preferred experience includes: Bachelor’s Degree in nursing preferred.  Three years’ experience in a psychiatric setting, with at least one year’s experience in an outpatient setting is preferred. $40.16/hour.  Schedule: 16 hours per week (no benefits).  Work hours are within the client service hours of Monday-Thursday, daytime hours.

Overnight Counselors Shift Supervisor – Full Time – working in the community residence/group homes setting providing supervision, training, guidance to the program staff during the overnight shifts. Complete periodic checks on Overnight Counselors and ensure Overnight Counselors are performing all assigned tasks including medication counts and ensure the group home is maintained and a clean environment for our clients. Provide support and guidance to Overnight Counselors by regularly communicating with them and respond to their calls. Provide overnight shift coverage as needed when there are call-ins and/or vacancies that cannot be filled by finding coverage from the list of employees available or doing the shift yourself. Regularly communication with Program Directors.  Know and follow agency program procedures and policies and provide guidance to all staff regarding these policies and procedures.  Min qual include: An acceptable combination of relevant education and exp including a bachelor’s degree and one year’s exp, associate degree and 3 years’ exp, or a high school diploma and 5 years’ exp., mental health exp preferred, must have a valid and insurable driver’s license; supervisor exp also preferred. 40 hours per week. Salary is $24.30/hour, $50,544.00/annual equivalent. Overtime is available; $36.45.

Assistant Program Director at 1574 House- Full Time – 1574 Union St. Residence is a group home serving clients with a mental illness located in Schenectady, NY.  Duties include assisting the Program Director with the oversight of the daily operations of the group home, client services including teaching of daily living skills that is client-centered, staff supervision, and overseeing a safe, healthy, and clean environment for our clients.  Qualification include BA/BS in human services and 1-year experience in human services, Associates degree in human services with 3 years’ experience, or a combination of equivalent experience and education. Schedule: Monday-Friday, 8:30-4:30.  No after hours on-call.  Overtime is available in our group home setting, $32.81/her hour for overtime pay. 

Service Coordinator Full-Time: – Providing supportive counseling to adults who have a psychiatric diagnosis and who live in independent apartments in the Schenectady community.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $21.70/hour; $45,136.00/annual equivalent. On-Call for client emergencies on a rotating bases (approximately once every 10 weeks) for 1 week; $150 bonus for on-call.  Schedule-Monday-Friday, daytime hours.

Relief Counselor/Respite – Per Diem– Mohawk Opportunities, a small private, nonprofit located in Schenectady is seeking per diem (substitute) Relief Counselors to work in our four group homes. We serve adults who have a mental illness and are working on goals to transition to an independent setting. The clients we work with are very high functioning and are in the community independently. We DO NOT provide personal care and we DO NOT do physical restraints. All the services to our clients are verbal supportive counseling and teaching of daily living skills. You must be available for all shifts. Driver’s license is not required but preferred. Regular rate $20.50/hour and overtime rate is $30.75. We provide training and a supportive work environment. Flexible hours. You sign up for available shifts.  Must be able to work some evening, weekend, and overnight shifts. Must work one 8-hour shift each month.
