Outreach Case Manager – Full Time – Engage individuals identified by the Schenectady County Outreach HUB and assessing for eligibility and needs. Complete referrals to community resources and ongoing assessment process.  Work towards resolution of long-standing issues that affect smooth delivery of care, individuals’ stability in the community, and health promotion. Provide weekly intensive case mgt until they have been connected to permanent Health Home Care Management agencies. Provide comprehensive transitional care from inpatient to other settings to ensure individuals are linked with necessary services upon discharge from an inpatient setting or a transition to another service provider. Monitor service delivery and ongoing engagement.  Advocate for additional services as needed.  Min qual include 1 year exp. case mgt preferred and knowledge of mental illness, substance abuse, physical/medical health, and homelessness is required.  A valid and insurable NYS driver’s license.  Must have own vehicle to use for business purposes.  $21.87/hour; $45,489/annual equivalent. 40 hours per week.  Schedule can be flexible with 4 days at 10 hours/day.  Must work 1 weekend day.

Job Coach-  Full Time The Job Coach will collaborate with the Assertive Community Treatment Team (ACT Team) and other agency programs to identify individuals expressing a desire to work and to support these individuals to achieve successful employment.  This includes development, planning and implementation of individualized employment opportunities using the evidence-based Individual Placement and Support (IPS) model.  Assess strengths, employment experience, and ssist with resume writing, completing applications, scheduling job interviews and support individuals during the interview process or on the job using a harm reduction and trauma-informed approach.  Services will be provided in an individual’s home, community, places of employment or office.  Cultivate relationships with employers to develop a pipeline of opportunities. Provide transportation to individuals when necessary, using agency vehicles. Min qaul include: Associate degree/Bachelor’s in human services preferred; high school diploma/equivalent with relevant experience will be considered, 2 years’ exp. with job placement/coaching exp. with individuals experiencing mental health challenges. Exp. in building relationships and securing job placement with employers. Valid and insurable driver’s license.  Must have own vehicle to use for business purposes.  Agency vehicles are available for transporting individuals. $24.00-$26.00/hour based on experience. Annual equivalent of $49,920-54,080.  40 hours per week.  Monday-Friday, daytime hours with flexibility.

Assistant Program Director at 1574 House, Full Time –1574 Union St. Residence are group homes serving clients with a mental illness located in Schenectady, NY.  Duties include assisting the Program Director with the oversight of the daily operations of the group home, client services including teaching of daily living skills that is client-centered, staff supervision, and overseeing a safe, healthy, and clean environment for our clients.  Qualification include BA/BS in human services and 1-year experience in human services, Associates degree in human services with 3 years’ experience, or a combination of equivalent experience and education. In addition, experience working with the mentally ill population and residential experience required. Supervisor experience preferred. Valid and insurable driver’s license required.  $24.30/hour: $50,544.00/annual equivalent. Overtime is available; $36.45/overtime rate. 40 hours per week.

Dominion House Client Services Coordinator (Case Manager) – Full TimeProvide service plan development and case management services to clients at Dominion House.  Coordinate all collaborative meetings with counselors and clients to facilitate residential treatment plan meetings then write individual service plans using the a client centered approach.  In addition, provide case management services to clients including benefit entitlements.  Qualifications include BA in human services or combination of degree and experience.  Excellent written skills, communication skills, and computer skills required along with mental health exp.  A valid and insurable driver’s license also required.  40 hours per week.$22.81/hour; $47,444.80/annual equivalent. Overtime is available; $34.22/overtime pay rate.

Housing Case Manager – Full Time: – Provide case management services; securing safe and affordable housing; teaching the necessary skills to maintain stable housing; developing linkages to community services; and develop support plans for adults who have a diagnosis of mental illness, and mentally ill adults who need intensive support and engagement in services, (MRT) and HIV/AIDs, and/or unhoused.  College degree in human services and/or equivalent experience in the human service field especially in the populations diagnosed with mentally illness, substance abuse, and/or chronic homelessness.  In addition, case management experience and a valid and insurable driver’s license are required. 40 hours per week.  $21.87/hour.  $45,489.60/ annual equivalent. Schedule: Monday-Friday, 8:30-4:30.  No after hours on-call.  Overtime is available in our group home setting, $32.81/her hour for overtime pay. 

Service Coordinator Full-Time: – Providing supportive counseling to adults who have a psychiatric diagnosis and who live in independent apartments in the Schenectady community.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; 6 months-1year experience in human services preferred; valid and insurable driver’s license. 40 hours per week; $21.70/hour; $45,136.00/annual equivalent. On-Call for client emergencies on a rotating bases (approximately once every 10 weeks) for 1 week; $150 bonus for on-call.  Schedule-Monday-Friday, daytime hours. 

Residence Counselors–Full Time- Provide supportive counseling to adults with a psychiatric diagnosis living in a group home setting.  Duties include teaching of daily living skills including medication management and coping skills, crisis intervention and prevention, and maintaining a healthy and clean environment for our clients to become independent.  Qualifications include college classes in human services, BA preferred; demonstrated ability in helping others, valid and insurable driver’s license. $20.50/hour, 40 hours per week. $42,640.00/annual equivalent. Overtime rate is $30.75/hour.
