Supported Housing Program
The Supported Housing Program aims to provide affordable housing and community-based support to individuals with severe and persistent mental illness. The program is intended to assist individuals in their journey toward independent living in the community. This program offers less intense services compared to the Certified Apartment Program, focusing on providing support, encouragement, and reinforcing skills related to community living. The program utilizes scattered site rental units in the City of Schenectady with the capacity to serve 42 individuals at any given time. The level of service provided by staff is tailored to the specific needs of each individual.
About the Program
Five (5) of the Supported Housing Program opportunities are designated for young adults aged 17 to 23 who have been diagnosed with a major mental illness. In addition to assistance in securing affordable housing and ongoing support, these individuals receive specialized training in independent living skills, including budgeting, household management, shopping, meal preparation, and more.
The program staff collaborates with other community service providers to address the identified needs of all individuals served, such as mental health and/or substance abuse treatment, health care, public assistance, etc. An individualized service plan is developed with and for each resident when they enter the program. This plan is used to guide service delivery and help prepare residents for the transition to a more independent living situation.
To be eligible for admission to the Supported Housing Program, individuals must meet the following criteria:
17 years of age or older
A primary diagnosis of a psychiatric illness
Not currently assessed as being dangerous to self or others or otherwise in need of hospitalization
In adequate health so as not to require skilled nursing care
Voluntarily agree to comply with Mohawk Opportunities, Inc. Residency Agreement
Must demonstrate a willingness to assist in securing necessary funds to cover the cost of care and services
The Supported Housing Program by Mohawk Opportunities offers affordable housing and community-based support to individuals who have a history of mental illness, are HIV positive, and are homeless. This program is funded by Continuum of Care Services. The main goal of this program is to help individuals secure an apartment and acquire the necessary skills to maintain permanent housing in the community. The program utilizes single rental units in the City of Schenectady and has the capacity to serve up to 28 individuals at a time, including families. The level of service provided by staff varies based on the unique needs of each individual or family.
Program staff collaborate with other community service providers to ensure that the identified needs of all individuals served are addressed, i.e., mental health and/or substance abuse treatment, health care, public assistance, etc. Upon entering the program, an individualized service plan is developed with and for each resident and used to guide service delivery.
Referrals to the Continuum of Care component of the Supported Housing Program can be made directly to the program by calling the Program Director. Once received, all referrals are processed within 72 hours.
To be eligible for Continuum of Care services, individuals must meet the following criteria:
18 years of age or older
A primary diagnosis of a psychiatric illness or HIV/AIDS
Documentation of “homelessness” as defined by HUD.
Not currently assessed as being dangerous to self or others or otherwise in need of hospitalization
Voluntarily agree to comply with Mohawk Opportunities, Inc. Residency Agreement
Must demonstrate a willingness to assist in securing necessary funds to cover the cost of care and services
Continuum of Care
Rental Assistance Program
The Supported Housing Program runs a Rental Assistance Program for Individuals Living With HIV/AIDS. In addition to personalized support, the program offers monthly nutrition classes and regular socialization/recreation activities. It provides single rental units in the City of Schenectady and can accommodate up to 13 individuals at a time, including families. The level of service from staff varies based on the needs of each individual or family.
Referrals to the Rental Assistance Program component of the Supported Housing Program can be made directly to the program by calling the Program Director. Once received, all referrals are processed within 72 hours.
To be eligible for the Rental Assistance Program, individuals must meet the following criteria:
18 years of age or older
A diagnosis of HIV/AIDS
Voluntarily agree to comply with Mohawk Opportunities, Inc. Residency Agreement
Must demonstrate a willingness to assist in securing necessary funds to cover the cost of care and services
Participation in any of the Supportive Housing Programs is voluntary.
No person shall be excluded from admission to the program based on race, religion, ethnicity, age, gender, sexual orientation or HIV status.
